Any individual who participates in any Gymnastic instruction (including Birthday Party participants) at Gold Medal Gymnastics, Inc. is required to have a Waiver & Release form on file. Without a Waiver & Release form, you will not be permitted to participate.

The Waiver & Release form requires the Adobe Acrobat Reader.
